  • SNHD: You can apply online through the SNHD website. 
  • DPBH: The Nevada Division of Public and Behavioral Health (DPBH) also offers online ordering through VitalChek. 
2. Mail-in Application:
  • SNHD: Download the application form from their website. Mail the completed form, a copy of your ID, and the fee to the address specified on the form. 
  • DPBH: You can mail your application to the Nevada Office of Vital Records in Carson City. 
3. In-person Application:
  • SNHD: Visit the Vital Records Office located at 280 S. Decatur Blvd, Las Vegas, NV 89107 during business hours (Monday-Friday, 8:00 am to 4:30 pm). 
  • DPBH: You may also be able to schedule an appointment at the Carson City office. 
Important Information:
  • VitalChek: You can use VitalChek for expedited online ordering of birth certificates through the Nevada Division of Public and Behavioral Health. 
  • Fees: The fee for a certified copy of a Nevada birth certificate is $25. 
  • Identification: You’ll need to provide a copy of a valid photo ID. 
  • Proof of Relationship: If you are requesting a birth certificate for someone other than yourself and you are not listed on the certificate, you may need to provide proof of relationship.
